Apparently, you worked at Motel 6 in calendar year 06. Motel 6 may have sent your W2 to your last known address and may have moved in without leaving a forwarding address.

Without threatening anyone, you should go to your employer and ask for a mailing address from the motel list. Then write a good letter to the provider informing you that you need a new W2.
